Microsoft helped explain what is most likely going on:

According to the Microsoft Advertising Help, when you have a call asset and a location asset, the location asset is where the call extension is actually showing. In reality, we have been seeing the phone call conversion tracking being quantified on both call assets and location assets in some of our Google Ads accounts. We also believe this could be some of the recent disapproved phone numbers in accounts, marked as an “unverified phone number” in the google ads call asset/ extensions tab . As the Google Ads Help pages is missing this information, we hope it can help others better understand the current metrics they are seeing in their accounts.
